Terms and Privacy Collection Notice for Doctor Applications
About these terms
These terms apply when you use this form to apply to work with PREXA PTY LTD (ABN 35 639 109 827), trading as SISTR ("we", "us"). Please read them, and the privacy collection notice below, before you continue.
Using this form
- You must be a medical practitioner registered, or eligible for registration, with the Australian Health Practitioner Regulation Agency (AHPRA).
- The information you give must be true, accurate and your own. Giving false or misleading information may end your application, or any engagement that follows it.
- Submitting an application does not create an offer, a contract or an obligation on either side. Any engagement is made only by a separate written agreement.
- We may close, pause or change this form, or the roles it applies to, at any time.
- Do not upload anything other than your own CV. Files are checked for malware, and a file that fails the check is deleted.
Privacy collection notice
Who is collecting your information
PREXA PTY LTD collects the personal information you enter in this form. You can contact our privacy officer at info@sistr.com.au.
What we collect
- Your name, email address and mobile number.
- Your AHPRA registration number and the area of practice you choose.
- Your CV and anything else you choose to tell us.
- A record of how far you got through this form and when: which steps you completed and which information pages you viewed.
- Technical details for security: your IP address and browser details when you accept these terms, and a security check provided by Cloudflare Turnstile to tell people from automated programs.
Why we collect it
- To assess your suitability for a role with us, and to contact you about your application.
- To check your registration on the public AHPRA register.
- If your application is successful, to set up your engagement with us. Your details then move into our practice management system and are handled under our privacy policy as a contracted doctor.
- To keep this form secure and to understand where applicants stop, so we can improve it.
Information you have not submitted
We keep what you enter even if you do not finish or submit your application. We do this so you can continue later, so we can follow up with you, and so we can see where the form is not working. If you do not want us to keep it, contact us and we will delete it.
Following up an unfinished application
If you stop part way through, we may email you a link to continue, and a recruiter may contact you by email or phone to follow up. Every follow-up email includes a way to tell us to stop, and we will not contact you about the application again after you do.
If you do not give us this information
We cannot consider an application without your name, contact details, AHPRA registration number and CV.
Who we share it with
We do not sell your information. We share it only with:
- our staff who are responsible for recruitment;
- service providers who host and run this system and send its emails, including Amazon Web Services and Cloudflare, under contracts that require them to protect it;
- AHPRA, when we check the public register; and
- anyone we are required or authorised by law to tell.
Your information is stored in Australia. Cloudflare's security check may process technical details (such as your IP address) outside Australia.
How long we keep it
- An application you did not submit: 24 months after your last activity, then deleted.
- An unsuccessful or withdrawn application: 24 months after it closes, then deleted, except for anonymous statistics.
- A successful application: kept as part of your record as a contracted doctor.
Your rights
You can ask to see or correct the personal information we hold about you, or ask us to delete it, by contacting our privacy officer. Our privacy policy, at www.sistr.com.au/privacy-policy, explains how to do this and how to complain about how we have handled your information. If you are not satisfied with our response, you can complain to the Office of the Australian Information Commissioner (www.oaic.gov.au).
